How our process works:

Step 1: Fill out the enrollment form below so that our team can coordinate in accordance with your personalized program.

Step 2: Your random notifications will be sent to you via email, once your program begins. By default, notifications will be sent out one day prior to the testing date, unless your program says otherwise. To fulfill the requirements of your program, you must complete the test by the deadline. Failure to do so will result in consequences. Please be mindful that your date is selected at random by a computer-generated system.

Step 3: Once you have received your notification, your payment link to make a payment for testing will be included, along with the details of the date you were selected. There is a form in the email that you must complete each time you are notified which will prompt you to the information we require to create your barcode authorization to walk into the facility.

Step 4: You will receive a final email that has your barcode authorization to complete the test which will include the location details and test type.

Step 5: Results will be emailed to the designated email on file by our partner Precision Mobile Lab Testing.
