On-Site Assistance

  • Prepare your materials at least one day prior to on-site date. Prefill chain of custody forms as well

    Supplies

    • Suitcase

    • Follow any protocols that company may have requested on paperwork.

    • Call the company at least 15 minutes before arrival to let them know you are arriving soon, if you are arriving late, communicate to the company before their actual time

    • Once you arrive, ask for the on-site coordinator and greet them with your name and our company name

    • Ask if you can be placed near their restroom for set up

    • Let the client know it will take about 5-10 minutes for set up

    • Let the client know how many you can take at a time and the employees should have an ID or work ID, otherwise a manager can identify the employee

    • Unload the correct drug cups on table set up (you can put a few at a time on desk area and place more when needed

    • set your pens, blue dye, gloves, paperwork

    • Check the restrooms to make sure anything nearby won’t be used to tamper the specimen.

  • 1.       GRAB CORRECT TEST DOCUMENTATION-

    2.       IF USING INSTANT SHEET - place a few lab copies nearby and grab your pens and black sharpie

    3.       The instant sheets should have the company information pre-filled

    4.       Coordinate so you don’t have multiple people in your face, communicate that you just need up to 3 people to check in with you and must have their ID’s.

    5.       Ask if those who are checking in with you are ready to use the bathroom, If not, then allow those who are ready to check in with you and advise those who aren’t ready to drink at least one bottle of water.

    6.       COLLECT EMPLOYEE IDS- Employees should present their ID to you: If no ID is present, they can use their employee ID, or an employer can verbally verify them.

    (If multiple people check in with you): Collect all IDs and have them on standby so you can fill out their paperwork. (Take at least up to 5 IDs maximum at a time)

    1. Script: Attention, I need at least 3 people to check in with me who are ready to use the restroom. Please have your Ids out…

       If you do not have your ID, your employer can verify you with me.

  • 1.       Fill out the donor information with the employee’s ID’s (Phone number can be collected later) and pace IDs on top of their paperwork

    2.       Next section: Check mark verified by:

    3.       Reason for test should be pre-filled

    4.       Take your sharpie and If the cup is not marked already, mark it to the 45ML and write the first names of the employee on top of the cup.

    5.       Have the employee(s) remove all their belongings in any pocket they have. Money and their wallets can remain with them.

    Script: “If I can have you remove all your belongings out your pockets and place your items in the desk area. Money and wallet can remain with you.”

     

    6.       Explaining process to employees:

     

    Script: “When you head into the restroom, the minimum that you will be providing me is up to the black mark on the cup. You will NOT FLUSH or NOT WASH your hands until after you hand me the cup.

     

    7.       Place gloves on and have the employee(s) follow you to the bathroom.

  • Beginner- 1-3 people at a time

    1 bathroom with one stall: You can have 3 people check in with you and start the paperwork and explain everything but have each employee go in one by one to the restroom with you while the others wait to go in next. Follow each employee one by one towards the bathroom.

    1 bathroom with several stalls or several bathrooms: If there is more than one stall, you can take as many as how many stalls they have.

    Gender based restrooms:

    If the employees are mixed male and female, it is ok to take both as long as the restrooms are nearby each other. If both bathrooms are far apart from each other, take one gender at a time in rotation.
